Your initial appointment will consist of a thorough oral examination, followed by a complete consultation explaining your diagnosis and treatment options. Depending upon the complexity of your case and the doctor’s professional recommendation, treatment may begin at the time of the initial consultation.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip (given to you by your general dentist) and any X-rays (if applicable).
  • A list of medications you are presently taking along with a list of medications you may have allergic reactions to.
  • If you have dental insurance, please bring your insurance card and any necessary forms to save time and allow us to help you process any claims.

For your convenience, a medical history form may be downloaded and printed from this Web site. This form can be completed prior to your first appointment and brought into the office. To download this form, go to the online forms section of this website.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the time of the consultation visit. Please alert the office if you have a medical condition or are taking any medications, which may have an affect on your endodontic treatment.

X-Ray Safety and Information

Today, more than ever, patients are concerned with the safety of dental x-rays and their benefits. Radiation, in the amounts used to process dental x-rays, is very small and thus provides minimal exposure and risk to the patient.

If your dentist has previously taken x-rays, you may request that they forward them to our office and if additional x-rays are necessary, they can be taken at our facility

Our offices use digital radiography, a new technology which greatly reduces the radiation dosage to our patients. If you would like more information on this new technology, please visit www.kodakdental.com.

However, at times, the use of traditional film radiographs are sometimes required. Our offices use ultra-speed film, which requires as little as one tenth of a second exposure. This type of x-ray permits the lowest level of radiation possible while still producing a clinically acceptable diagnostic x-ray.
